Support plans and risk assessments are regularly reviewed to ensure support remains appropriate and proportionate to the individual’s changing needs.

We recognise that some individuals with a forensic history may have experienced difficult relationships with services and may require time to develop trust.

Our staff provide consistent, respectful and non-judgemental support, focusing on the individual rather than their history. People are actively involved in decisions about their support, goals and future.

We promote a least restrictive approach, helping individuals exercise choice and independence while ensuring identified risks are appropriately managed.

Safe Transitions into Supported Living

Moving from a secure or highly structured environment into the community requires careful planning.

Divine Focus Care Group Ltd works with the individual, commissioners, families where appropriate, and relevant professionals to understand their history, support needs, risks and desired outcomes before a placement begins.

Where appropriate, we can support a planned transition through introductory visits, meetings with staff, familiarisation with the service and a phased transition into the placement.

Working in Partnership

Effective forensic support requires strong multidisciplinary working. We work collaboratively with relevant professionals and agencies, which may include:

  • Local authorities and commission
  • Probation services
  • Forensic and community mental health teams
  • Social workers
  • Psychologists and psychiatrists
  • Other health and social care professionals
